Abstract

Closure of the left atrial appendage with a device facilitates the prevention of stroke in patients with a history of nonvalvular atrial fibrillation and a contraindication to the use of anticoagulant medications. This article presents two cases of patients with a history of atrial fibrillation at the Calderón Guardia Hospital, both with a history of digestive bleeding and contraindication to anticoagulation, in whom closure of the left atrial appendage was achieved through successful placement of the Amulet™ device, using CT and Heart Navigator, imaging techniques that are becoming the gold standard in many international reference centers in interventional cardiology.

Abstract

Mural endocarditis is an uncommon form of intracardiac infection affecting the non valvular endocardium that can present with complications similar to valvular infective endocarditis. Echocardiography is recommended to confirm the diagnosis when there is a high index of suspicion. Regarding treatment, there is limited evidence about therapeutic strategies in mural endocarditis, however in most reported cases it is recommended to initiate targeted antibiotic therapy associated with early surgical intervention. The following is a clinical case of a 74-year-old male patient with systemic embolic phenomena, in whom a transesophageal echocardiogram documented mural endocarditis in the apex of the left ventricle associated with Staphylococcus aureus bacteremia. This case highlights the importance of a detailed echocardiographic assessment of the cardiac valves and chambers when infective endocarditis is suspected.

Abstract

Protocol proposal for exercise prescription in cancer patients or cancer survivors for the prevention of cardiovascular disease

Cardiovascular diseases and cancer correspond to the first and second causes of death worldwide. They share common risk factors: aging, unhealthy lifestyles, among others; with the attenuating circumstance that cancer patients or survivors may develop cardiotoxicity due to their treatment. Therefore, the prevention and treatment strategies in some of its approaches are also similar. Diseases can be considered simultaneous or consecutive, they can coexist in the same patient, so a multidisciplinary approach is appropriate, based on risk assessment, physical training, optimization of drug therapy, healthy lifestyle counseling, control of cardiovascular risk factors, psychological support, adherence to a more active lifestyle with the consequent improvement in cardiopulmonary and therefore functional capacity. This document intends to propose a protocol proposal for the prescription of exercise in cancer patients or cancer survivors for the prevention of cardiovascular disease, based on international consensus recommendations.
